Resumen
Este estudio observacional retrospectivo evaluó la tasa de infecciones asociadas al uso de catéteres en pacientes sometidos a terapia de recambio plasmático en un centro de aféresis en Colombia. Se analizaron los registros de 133 pacientes entre 2021 y 2024. Se identificó una tasa de infección baja (1 por cada 1000 días-catéter), con 4 casos confirmados, todos en mujeres jóvenes con enfermedad renal crónica y catéteres tunelizados. La mayoría de los pacientes recibió cuidados adecuados según protocolo, lo que, junto con medidas de prevención como curaciones periódicas, uso de apósitos estériles y adaptadores sin aguja, contribuyó a los buenos resultados. El estudio resalta la importancia de implementar estrategias estrictas de control de infecciones en unidades de aféresis.
Abstract
Catheter-related infections are a common and potentially serious complication in patients undergoing therapeutic plasma exchange. However, the incidence of these infections and their associated risk factors have not been thoroughly studied. Objective: This study aimed to determine the rate of catheter-associated infections in patients undergoing therapeutic plasma exchange at an apheresis center in Colombia, as well as to identify the risk factors associated with these infections. Methods: An observational, descriptive, and retrospective study was conducted involving 133 patients who underwent therapeutic plasma exchange between January 2021 and December 2024. Data were collected on demographic and clinical characteristics, type of catheter, therapy duration, presence of comorbidities, and additional medical devices. The infection rate was calculated as the number of infections per 1,000 catheter-days. Results: A total of 4 confirmed infections were documented, corresponding to a rate of 1.0 infections per 1,000 catheter-days (95% CI: 0.27–2.56). All infections occurred in tunneled catheters and exclusively in female patients, with a mean age of 28.3 years. Among the infected patients, 75% had chronic kidney disease and 50% had additional medical devices. The mean duration of therapy was significantly longer in the infected group (74.8 days) compared to the non-infected group (28.7 days). Conclusion: The rate of catheter-associated infections in patients undergoing plasma exchange therapy at the studied center was low. Chronic kidney disease, the use of additional medical devices, and longer catheter duration emerged as relevant associated factors. The implementation and maintenance of strict infection prevention strategies remain essential to minimize the risk of infections in this patient population
