Asignación de recursos en el sector salud colombiano: una revisión sistemática de literatura

Resumen
Objetivo: Revisar la evidencia disponible sobre la asignación de recursos en el sector salud colombiano, esto desde las diferentes perspectivas de análisis en evaluación económica en salud en el periodo comprendido entre 2010 -2020. Método: Se llevó a cabo Revisión Sistemática de Literatura -siguiendo el protocolo PRISMA y con la verificación de estándares de calidad mediante la metodología CHEERS -Consolidated Health Economic Evaluation Reporting Standards. Resultados: Según el resumen de valoración, tres de los artículos elegidos para la revisión sistemática, que representan el 60 %, tienen una calidad general aceptable, esto quiere decir que la valoración general, obtenida de sumar las puntuaciones ponderadas los resultados, se ubicó entre 35-69 puntos, mientras que el 40 % restante tuvo una calidad general dudosa, es decir, la valoración general de estos artículos se ubicó entre 14-34 puntos. Entre los hallazgos se hace evidente la escasez de literatura que soporte el objeto central de la presente investigación. Sin embargo, en los artículos seleccionados para esta revisión, y luego de aplicada la lista de chequeo para determinar la calidad metodológica establecida por CHEERS, se vislumbran diferentes aportes que afectan la asignación de recursos en el sector. Conclusiones: Con esta revisión se puede concluir que las evaluaciones económicas centradas en la aplicabilidad, en cuanto a la asignación de recursos en el sistema de salud colombiano, son limitadas, por ende, el número de factores identificados podrían no ser lo suficientemente precisos o no contener la amplitud real del tema.
Abstract
Objective: To review the available evidence on the allocation of resources in the Colombian health sector from the different perspectives of analysis in health economic evaluation for the period 2010-2020. Method: A review was carried out following the application of the Systematic Literature Review - PRISMA protocol and verification of quality standards using the CHEERS -Consolidated Health Economic Evaluation Reporting Standards methodology. Results: The evaluator determined that of the articles chosen for the systematic review, 60% (3 of the articles), according to the assessment summary, presented an acceptable overall quality, which means that the overall assessment obtained from adding the weighted scores, the results were between 35-69; in this order of ideas, 40% of the articles obtained a doubtful overall quality, that is, the overall assessment of these articles was between 14-34 points; Among the findings, the scarcity of literature that supports the central object of this research is evident; however, in the articles selected for this review, after applying the checklist to determine the methodological quality established by CHEERS, different contributions that affect the allocation of resources in the sector can be glimpsed. Conclusions: This research concludes that there are limited economic evaluations that focus on the applicability in terms of resource allocation in the Colombian health system, and therefore the number of factors identified may not be sufficiently precise or contain the real extent of the issue in question.
