Alberto; GuzmĆ”n, G.Purpose: To evaluate the quality of life in patients 65 years or older withactive epilepsy in two hospitals in Bogot?, Colombia.Method: The clinical charts including the electroencephalogram anddiagnostic images of 120 patients were reviewed. Then the QOLIE-31and version 10 of Minimental Tests were used to establish the patient'squality of life score and his cognitive level. Global quality of life and sub-scales scores were correlated with quantitative variables using the Spear-man coefficient and differences between qualitative variables wereevaluated with non-parametric tests. To establish the variables thatexplained the quality of life, we used an ordinal regression model. Thisstudy was approved by the Research Ethics Committee of both hospitals.Result: 106 patients of both sexes with an age of 71.9ā¬6.32 years werestudied. Epilepsy began at 61.92ā¬16.5 years and only 4% of patients wereseizures free for the past two years. 88.7% had focal seizures and if anunderlying etiology was identified, cerebral vascular disease was pre-dominant. 71% were on Phenytoin monotherapy. Global quality of lifescore was 52.17ā¬15.83. It was affected by number of seizures, Mini-men-tal test score, educational level, socioeconomic status and MRI abnor-malities. Other variables such as gender, housing site, job, type ofseizures, availability of AED and treatment with phenytoin influencedQOLIE subscales.Conclusion: Quality of life is an individual perception that could beinfluenced by many factors. Patients over 65 years of age with epilepsyusually show a poor quality of life not only related to the disease but alsoto their socioeconomic conditions

Sjƶgrenās syndrome (SS) is considered an AD affecting mostly older women. However, information about how does AOD influence the course of SS is scarce.

But we really don't know whether we have done it right, or are doing it better, unless we have a measurement system for quality.' Unfortunately, there is little agreement in the standardization of indicators used for evaluation of organizations related to medical equipment management. And those that do the first steps, always walk on thin ice. With this paper, we suggest a set of five quality indicators for the control and the evaluation of management for medical equipment maintenance. The indicators proposed allow the organization, that applies them, to easily correct and adjust their management programs, with a strive for improvement in results and quality and broadening their experiences. The selection of the indicators was executed according to those that are most used among leading health care organizations. Sometimes the indicators are labeled differently but the basic idea is the same, and therefore the results can be compared competitively and the potential of an organization can be displayed.

While an OMS report in 2008, the mortality of perinatal asfhyxia in Colombia was about 7%. The newest report from 15 to 20% of the newborn that have asfhyxia during this moment or in a short period near, death during neonatal period and those who survived, 25% have permanent neurological damage. One of the quality standard about Health Service is to recognize the population caractheristic, those evidence guarantees the required information with the purpose about right decision taking a measure designed to improve diagnostic and prognostic of that population.

AdemĆ”s de sus graves complicaciones como la falla renal crónica y la retinopatĆa diabĆ©tica, suelen asociarse otras comorbilidades que incrementan el riesgo clĆnico y la polifarmacia correspondiente. Objetivo: DiseƱar un modelo de gestión de riesgos económicos y epidemiológicos a partir del anĆ”lisis de prevalencia por gĆ©nero, diagnóstico y costos hospitalarios asociados, de la Diabetes Tipo II y sus comorbilidades en Colombia. MetodologĆa: A partir de los datos de hospitalización y costes asociados del Hospital Universitario Mederi de BogotĆ” para los aƱos 2017-2018. Se realiza un estudio descriptivo transversal de costos por tipologĆa. Se analizan las diferencias estadĆsticamente significativas de estancias y costes medios por gĆ©nero, morbilidad y diagnósticos. Resultados: El Hospital Mederi tuvo un total de 41.113 egresos en 2017 y 45.605 en 2018 con un promedio de estancia 6.8 en 2017 y 6.4 en 2018. Los pacientes diabĆ©ticos suponen aproximadamente el 25% y presentan una estancia media de 5 dĆas. Se observan diferencias significativas de gĆ©nero en la estancia media. El gasto total estimado para pacientes diabĆ©ticos, con y sin comorbilidades asociadas, fue de $ US 2.204.860 (30,1%) para 2017 y de $ US 2.192.365 (27,2%) para 2018. Aproximadamente, el 40% de los ingresos se deben a 20 diagnósticos principales que originan un 30% del coste total. Las principales comorbilidades asociadas a la diabetes, son: Infección de vĆas Urinarias, Hipertensión arterial, Insuficiencia cardiaca y Enfermedad IsquĆ©mica Coronaria. Conclusiones: El modelo de gestión de riesgos se basa en el estudio del gasto en servicios sanitarios oca- sionados por la diabetes y sus comorbilidades, Ć©ste impacta significativamente en el presu- puesto de los servicios de salud, siendo el gasto farmacĆ©utico el de mayor peso relativo. El gasto en hombres es mayor que en mujeres.

F.Purpose: To analyse systematically the content of potentially seizureprovoking video sequences from different Spanish television broadcast-ers checking for compliance with current guidelines in other countries.One in 4000 subjects is at risk of having a seizure when confronted withcertain visual stimuli, often without being aware of their condition. Tele-vision is the most common cause of photosensitive seizures in the wes-tern world, but only the UK and Japan have adopted guidelines to preventthe broadcasting of video sequences that could potentially trigger sei-zures in susceptible people.Method: Television programs of all 5 national broadcasters availablevia Digital Terrestrial Television were recorded between 8 AM and 12PM on 5 consecutive days, from January 1st 2012 through January 6th2012. In addition, a channel with programming dedicated to children anda musical channel were also recorded. The video content was analysedusing a HardingFPA Flash and Pattern Analyser (Cambridge Researchsystems) for compliance with the UK Ofcom guidelines.Result: We analysed 105 hours of television programs. We found 738incidents that violated the guidelines (mean 105.43, CI95% 58.74ā151.12), mostly luminance flashing (714 incidents, mean 102, CI95%55.83ā148.17). Thirteen incidents involved spatial patterns and 11 redflashing. Forty-seven additional incidents considered dangerous weredetected which, although technically meeting guidelines, continued foran extended period of time and were considered dangerous.Conclusion: Spanish broadcasters seem to be unaware of the risk ofphotosensitive epilepsy. National Guidelines should be adopted to lowerthe risk of Spanish TV content triggering epileptic seizures in susceptibleviewers

D.; Anaya, Juan-Manuel; Rojas-Villarraga, A.Background Erosions are the hallmark of rheumatoid arthritis (RA). Although most of erosions may occur within 2 years of onset of the disease, their progression does not correlate with the duration of RA. Moreover, information about non-erosive RA is ambiguous. Objectives To evaluate the prevalence and characteristics of non-erosive RA. Methods Since no agreement about the time-lag interval for erosions appearance in RA exists, and there is no universally accepted definition about it, a zero score in hands and feet X-ray at 5 years of RA duration (by SvHD) was arbitrary considered as cut-off. Therefore, patients meeting these criteria were searched in a large cohort of RA patients (ACR, 1987). Metacarpophalangeal and metatarsophalangeal joints as well as wrist ultrasound (US) imaging (by OMERACT), and computed tomography (CT) were performed to confirm non-erosive RA. Spearman correlation coefficient, kappa analysis and KendallĀ“s W test were used to analyze data. Results Out of a total of 800 RA patients, 38 met inclusion criteria (4.75%). Most of them were women (76.3%). The median (IQR) of the age and duration of the disease were 52 (13) and 10 (8) years, respectively. Rheumatoid factor (RF) and anti-CCP antibodies were positive in 86.8% and 55.3%, respectively. Non-erosive RA was confirmed by the three methods in only 7 patients. There was no concordance between the three methods. For this sample, the sensitivity and specificity were 70% and 52% for US, and 58% and 57% for CT, respectively. Non-erosive RA was positively although weakly correlated with improved functional class according to the HAQ (r=0.32, p=0.045) and ANA positivity (r=0.43, p=0.030), and negatively correlated with activity of the disease (by RAPID3, r= -0.32, p=0.05), the use of biological therapy (r= -0.31, p=0.05) and RF (r= -0.36, p=0.042). Conclusions Non-erosive RA is rare (0.87%). A good functional class, ANA positivity, RAPID3 score, the use of biological therapy and RF influence the absence of erosions in long-standing RA (i.e., 5 years). Since there is a high variability and a lack of concordance exists among the imagining techniques, a consensus and unified definition about non-erosive RA are warranted.
- ĆtemSolo Metadatos344 Point of Care Ultrasonography Training in BogotĆ” Emergency Medicine Residencies: The Unmet Need(2012-10) Henwood, P.C.; Norwood, B.R.; Genthon, A.A.; Silva, D.; Martinez, Y.B.; Granada, J.; Torres, J.; Romero, M.G.; Arbelaez, C.Background: As ultrasonography equipment has evolved toward increased portability, higher quality and lower cost, the field of point-of-care ultrasonography has grown, with numerous applications for emergency medicine practitioners. Competence in goal-directed ultrasonography image acquisition and interpretation is now a mandatory part of emergency medicine residency training in the United States and the United Kingdom. Appropriate training is a key component to the safe and effective use of this technology. In Colombia, emergency medicine is a relatively new specialty and leaders in the field have expressed interest in further integrating pointof-care ultrasonography into emergency medicine practice and resident training. Study Objectives: The objective of this study was to conduct a needs assessment to better understand the current state of point-of-care ultrasonography practice among all emergency medicine residency programs in BogotĆ”, Colombia. Methods: An ultrasonography needs assessment survey tool was distributed to all available residents of the 3 emergency medicine residency programs in BogotĆ”. The emergency medicine program directors, other physicians staffing the emergency departments, and several medical students rotating in the emergency departments were also surveyed. The needs assessment survey tool assessed the populationās experience with point-of-care ultrasonography, formal ultrasonography education, and interest in educational initiatives. Focus group sessions with 50% of emergency medicine residents in BogotĆ” were subsequently conducted to further qualify their perspective on the current state of diagnostic ultrasonography at each residency training site as it relates to their clinical and procedural practice. Partners Health care institutional review board exemption was obtained for this research. Results: Fifty four total clinicians (43 emergency physicians, 5 non-emergency physicians and 6 medical students) completed the survey. Twenty-nine participants (54%) had previously used an ultrasonography machine. Of all emergency medicine respondents, 24 had used ultrasonography, and trauma was the most common application. However, 52% of those who had used ultrasonography had performed less than 10 independent scans. Thirty-nine participants (72%) had received no formal ultrasonography training. Of respondents, 94% total (100% of emergency physicians) reported a high level of interest in learning point-of-care ultrasonography, particularly trauma and vascular access applications. The major barriers identified by those surveyed were the absence of teachers (83%), the absence of ultrasonography equipment (65%), and time limitations (39%). Focus group discussions revealed that the scope of clinical practice, especially regarding procedures performed by emergency physicians, varied between emergency departments and residencies, but all reported they wanted more training in the use of point-of-care ultrasonography to improve their practice and patient care. Conclusion: Emergency ultrasonography training and use in clinical practice among emergency medicine residencies in BogotĆ” is currently limited by the lack of ultrasonography equipment and educators. However, there is significant interest in educational initiatives among all residents and program directors. Further research into how to best integrate point-of-care ultrasonography into emergency medicine practice and resident training in Colombia is needed.

Therefore, the purpose of this study was to develop age-and sex-specific normative reference values for lower-body muscular power assessed by the standing broad jump (SBJ) in Colombian schoolchildren. METHODS: A total of 7244 children and adolescents (55.7% girls, with a sample age range of 9-17.9 years) completed the SBJ test (median age, in years = 12.8 (SD 2.3); 25th-75th percentile: 11.0-15.0. The distance between takeoff and the heel of the closest foot at landing was recorded in centimeters, and participants were allowed one more try if they landed with their hands behind their feet. Smoothed percentile curves and tables for the 3rd, 10th, 25th, 50th, 75th, 90th and 97th percentiles were calculated using Coleās LMS method. RESULTS: The one-way ANOVA tests showed that maximum SBJ (cm) was higher in boys than in girls (p<0.01). Post hoc analyses within sexes showed yearly increases in SBJ scores in all ages. In boys, the maximum SBJ scores 50th percentile ranged from 110 to 165 cm. In girls, the 50th percentile ranged from SBJ scores was 96 to 120. Both linear and quadratic age terms were statistically significant predictors of SBJ trends across age. CONCLUSIONS: Our results provide reference standards for sex- and age-specific SBJ scores in Colombian schoolchildren aged 9-17.9 years for the first time. The proposed reference values can be used to interpret SBJ scores in Colombian schoolchildren.

In Heliconius butterflies, a small number of genes control the development of diverse wing colour patterns. Here, we used full-genome sequencing of individuals across the Heliconius erato radiation and closely related species to characterize genomic variation associated with wing pattern diversity. We show that variation around colour pattern genes is highly modular, with narrow genomic intervals associated with specific differences in colour and pattern. This modular architecture explains the diversity of colour patterns and provides a flexible mechanism for rapid morphological diversification.

Contrary to the adult population, there is a huge knowledge gap regarding the epidemiologic and clinical characteristics of cardiomyopathies in children. This lack of information is even bigger when the situation of medical research in Latin America is taken into account. Given the potential impact that this disorders could have to health-systems, a comprehensive epidemiologic study regarding the clinical profile and sociodemographic characteristics of this subjects, will impact the way we approach paediatric cardiomyopathies. Methods: An observational, retrospective study was performed over a 7 years period, at a tertiary referral centre for Colombian and Latin American paediatric cardiology. We analyzed all cases of primary cardiomyopathy in children who presented to our institution between 2010-2016 and who were younger than 18-years of age. Cases of cardiomyopathy were classified according to World Health Organization guidelines. Results: From 29,533 children with suspected heart diseases that attended our institution during the study period, a total of 89 new cases of primary cardiomyopathy were identified. The median age at diagnosis was 11 years. Dilated cardiomyopathy made up 57.3% (N=51) of cases, hypertrophic cardiomyopathy 12.3% (N=11), restrictive cardiomyopathy 8.9% (N=8), non-compacted cardiomyopathy 7.8% (N=7), arrhythmogenic right-ventricular cardiomyopathy 6.7% (N=6) and unspecified cardiomyopathy the remaining 6.7% (N=6). Heart failure was present in 53.93% of patients. The overall mortality was 12.36% (N=11) including 2 of a total of 8 patients who received a cardiac transplantation. Conclusions: The findings in our study are in agreement with the current scientific literature on the frequency and gender distribution of the different groups of cardiomyopathies in the pediatric age. Currently we do not have any research in Colombia or Latin America that characterizes these patients and it is fundamental to carry out studies like this to understand the magnitude of the problem we face. It is very important to create multidisciplinary groups for the diagnostic and therapeutic approach of these patients, as well as to carry out new studies with a wider population.

Forty four (44) children (37 boys and 7 girls) were selected, mean age 14.4 ± 2.3 years old. The average workday was 9.25 ± 3.2 h/day. The most frequent activity was provisions and fruits retail sale (43.2%).

The fitness versus fatness relationship has not been studied in a population-based sample in Latin America. The aim of this study was to examine the relationship between the CRF with adiposity markers among children and adolescents from BogotĆ”, Colombia. METHODS: Cross-sectional study in 7,960 healthy Colombian youth (boys n=3,462 and girls n=4,494) residents in the metropolitan area of the District of BogotĆ”, Colombia (2480msnm). CRF was assessed with the 20-meter shuttle run test. Waist circumference (WC), body fat percentage-BF%, ? skinfolds thicknesses (triceps + subscapular), body mass index (BMI) and waist-to-height ratio (WHtR) were used as indicators of adiposity. RESULTS: After adjusting for sex, age and sexual maturation, significant inverse relationships were observed between CRF and BMI (r = -0.360), BF% (r = -0.387), ? skinfolds (r = -0.417), WC (r = - 0.258) and WHtR (r = -0.201), p < .001. CONCLUSIONS: Our results show that high aerobic capacity is associated with less adiposity markers. Increasing CRF should be targets in youth primordial prevention strategies of future cardio-metabolic disease in adulthood.

Breast-self examination (BSE) is an inexpensive strategy proposed to improve early detection behaviors. Behavioral risk factors are crucial for cancer control. More than half of breast cancer cases can be prevented through behavior change. Objectives To describe and establish assocations between the knowledge, attitudes and practices of breast-self examination, the knowledge about breast cancer risk and behavioral-related risk factors for breast cancer among female college students in BogotĆ”, Colombia. Methods A cross-sectional study was carried out involving data from 628 young female students. Data for BSE were collected using a validated tool and the Behavioral Risk Factors Surveillance System (BRFSS) was used for lifestyle-related variables. Results Women were a mean age of 21.7±11.8 years old. Fifty-seven percent of the women knew how to carry out BSE, although only 26.3 % perfom it monthly. Further, a sedentary lifestyle was found in 53.3 % of women and similar prevalences were observed for other behavioral variables. BSE was associated with age (p=0.02), socioeconomic status (p<0.001), knowledge about risk factors (p<0.001) and unhealthy lifestyles (p<0.001). Conclusions There exists a low level of knowledge and practice of BSE among female college students. This population is at a high-risk of breast cancer beacuse of the critical prevalences of some behaviors related to unhealthy lifestyles found in this study. Further health-promotion strategies are warranted.

The diagnosis of idiopathic PH is made after a thorough study clinical and paraclinical including complete anatomical and functional assessment. The aim of this study is to describe echocardiographic findings and hemodynamic data obtained by cardiac catheterization and assess their degree of correlation. Materials and Methods: Descriptive cross-sectional study and analysis of matching variables through the comparison of pulmonary pressure measured by echocardiography and catheterization through Interclase correlation coefficient (ICC) and the Bland Altman. The study population were children aged 0 days to 18 years who presented Fundación Cardioinfantil ā Instituto de CardiologĆa in BogotĆ” (Colombia) diagnosed with idiopathic PH evaluated between January 2010 and December 2013. Results: 29 patients diagnosed with idiopathic PH studied with echocardiography and cardiac catheterization were included. The degree of PH documented by echocardiography corresponded to severe in 38% of patients. After cardiac catheterization, the degree of PH was classified as severe in 66% of cases. In 18 patients (62%) pulmonary vascular reactivity test was performed, finding positive response in 7 cases (39%). The concordance analysis of pulmonary pressure measured by echocardiography and catheterization was performed in 20 subjects, with a confidence level of 95%. When comparing the measurement results in both tests is a difference of 15 mmHg which is statistically significant. The tests achieved an ICC of 0.55, with 95% of 0.16 to 0.79, which represents a force of moderate agreement. Conclusions: In the present study the severity of PH corresponded to severe in most patients evaluated for cardiac catheterization. Performing pulmonary vascular reactivity test for both diagnosis and accurate diagnostic decisions is essential. The concordance analysis of pulmonary pressure measured by echocardiography and catheterization through Intraclase Correlation coefficient and the Bland Altman documents a force of moderate agreement
