- ÍtemAcceso Abierto004 - A Novel Familial Case of Diffuse Leukodystrophy Related to NDUFV1 Compound Heterozygous Mutations(2013-07-30) Rivera-Nieto, C.; Mateus, H.; Ortega-Recalde, O.; Fonseca Mendoza, Dora Janeth; Patiño, L.; Restrepo Fernández, Carlos Martín; Van der Knaap, M.; Laissue, P.Introduction: Mitochondrial leukodystrophy due to complex I deficiency is an entity with high genetic heterogeneity, the mutations of the gene NDUFV1 being one of the causes of this disease. It is an autosomal recessive entity that causes a variable phenotype, from a fatal neonatal onset to neurodegenerative disorders in adulthood. Problem Studied: We report 2 siblings, 5 and 2 years of age, with nystagmus, ataxia, impaired consciousness, hemiparesis, hyporeflexia, and psychomotor regression. In brain magnetic resonance imaging, signal abnormalities in large regions of the cerebral white matter were observed, suggesting a demyelinating disease. Materials and Methods: Genomic DNA was obtained from whole blood. The complete coding region of NDUFV1 was amplified in patients and their parents. Each amplicon was purified. Direct sequencing was performed. Intron 6 and exon 7 amplicons from patient 1 were cloned. A skin biopsy was performed in the mother. RNA was isolated. The NDUFV1 complementary DNA (cDNA) was amplified and directly sequenced. In silico analysis was performed. Following diagnosis, treatment with ubiquinol and riboflavin was started. Results: Both patients have the missense mutation c.1156C> T and the 42-base pair deletion in the gene NDUFV1. Bioinformatics analysis indicates that this deletion leads to messenger RNA (mRNA) synthesis with a premature stop codon. Probably, mutant mRNAs were recognized and degraded by the nonsense-mediated mRNA machinery. Analysis of the maternal NDUFV1 cDNA supports this hypothesis. Conclusion: Our results add information on the molecular basis and the phenotypic features of mitochondrial disease caused by NDUFV1 mutations. We can affirm that the mutations are causative of the phenotype. The patients are having a good therapeutic response to the treatment.
- ÍtemSolo Metadatos344 Point of Care Ultrasonography Training in Bogotá Emergency Medicine Residencies: The Unmet Need(2012-10) Henwood, P.C.; Norwood, B.R.; Genthon, A.A.; Silva, D.; Martinez, Y.B.; Granada, J.; Torres, J.; Romero, M.G.; Arbelaez, C.Background: As ultrasonography equipment has evolved toward increased portability, higher quality and lower cost, the field of point-of-care ultrasonography has grown, with numerous applications for emergency medicine practitioners. Competence in goal-directed ultrasonography image acquisition and interpretation is now a mandatory part of emergency medicine residency training in the United States and the United Kingdom. Appropriate training is a key component to the safe and effective use of this technology. In Colombia, emergency medicine is a relatively new specialty and leaders in the field have expressed interest in further integrating pointof-care ultrasonography into emergency medicine practice and resident training. Study Objectives: The objective of this study was to conduct a needs assessment to better understand the current state of point-of-care ultrasonography practice among all emergency medicine residency programs in Bogotá, Colombia. Methods: An ultrasonography needs assessment survey tool was distributed to all available residents of the 3 emergency medicine residency programs in Bogotá. The emergency medicine program directors, other physicians staffing the emergency departments, and several medical students rotating in the emergency departments were also surveyed. The needs assessment survey tool assessed the population’s experience with point-of-care ultrasonography, formal ultrasonography education, and interest in educational initiatives. Focus group sessions with 50% of emergency medicine residents in Bogotá were subsequently conducted to further qualify their perspective on the current state of diagnostic ultrasonography at each residency training site as it relates to their clinical and procedural practice. Partners Health care institutional review board exemption was obtained for this research. Results: Fifty four total clinicians (43 emergency physicians, 5 non-emergency physicians and 6 medical students) completed the survey. Twenty-nine participants (54%) had previously used an ultrasonography machine. Of all emergency medicine respondents, 24 had used ultrasonography, and trauma was the most common application. However, 52% of those who had used ultrasonography had performed less than 10 independent scans. Thirty-nine participants (72%) had received no formal ultrasonography training. Of respondents, 94% total (100% of emergency physicians) reported a high level of interest in learning point-of-care ultrasonography, particularly trauma and vascular access applications. The major barriers identified by those surveyed were the absence of teachers (83%), the absence of ultrasonography equipment (65%), and time limitations (39%). Focus group discussions revealed that the scope of clinical practice, especially regarding procedures performed by emergency physicians, varied between emergency departments and residencies, but all reported they wanted more training in the use of point-of-care ultrasonography to improve their practice and patient care. Conclusion: Emergency ultrasonography training and use in clinical practice among emergency medicine residencies in Bogotá is currently limited by the lack of ultrasonography equipment and educators. However, there is significant interest in educational initiatives among all residents and program directors. Further research into how to best integrate point-of-care ultrasonography into emergency medicine practice and resident training in Colombia is needed.
- ÍtemSolo MetadatosA new proposal of quality indicators for clinical engineering: A New Beginning for Human Health: Proceedings of the 25th Annual International Conference of the IEEE Engineering in Medicine and Biology Society(2003-01-01) Miguel Cruz, Antonio; Rodriguez, E.; Sanchez, M.C.; Tolkmitt, F.; Pozo, E.As we know from Stiefels paper '...we want to do our work right the first time and better the next time. But we really don't know whether we have done it right, or are doing it better, unless we have a measurement system for quality.' Unfortunately, there is little agreement in the standardization of indicators used for evaluation of organizations related to medical equipment management. And those that do the first steps, always walk on thin ice. With this paper, we suggest a set of five quality indicators for the control and the evaluation of management for medical equipment maintenance. The indicators proposed allow the organization, that applies them, to easily correct and adjust their management programs, with a strive for improvement in results and quality and broadening their experiences. The selection of the indicators was executed according to those that are most used among leading health care organizations. Sometimes the indicators are labeled differently but the basic idea is the same, and therefore the results can be compared competitively and the potential of an organization can be displayed.

L.; Molano González, Nicolás; Mantilla, R. D.; Rojas-Villarraga, A.; Anaya, Juan-ManuelBackground Cardiovascular disease (CVD) is a major cause of morbidity and mortality in patients with systemic lupus erythematosus (SLE), a 50-fold than general population. Traditional risk factors, such as, cigarette smoking and dyslipidemia, are known but they cannot fully account for the increased risk. Instead, non-traditional risk factors, related to disease are also contributing. Varied results have been reported on the association between coffee drinking and CVD in the general population, with controversial results. Coffee consumption has not been evaluated systematically in SLE patients with CVD.

- ÍtemSolo MetadatosAntidepressants in painful diabetic neuropathy(2008-08) Gutierrez Alvarez, A.M.; Moreno, C.B.Background: The incidence of diabetic neuropathy increases withthe duration of diabetes and the degree of hyperglycaemia. Pain isone of the most common and incapacitating symptoms of diabeticneuropathy and its pharmacological control is complex. The effec-tiveness of antidepressive agents has been described in differenttypes of neuropathic pain, but their effectiveness when used as anal-gesics in painful diabetic neuropathy still remains controversial. The objective was to review the possible role of antidepressiveagents for the treatment of pain in diabetic peripheral neuropathy. Thus, this work has consisted of meta-analysis for determiningwhich antidepressive agent had the best analgesic potential for man-aging pain in patients suffering from painful diabetic neuropathy. Methods: The search covered the Cochrane, MEDLINE,EMBASE and LILACS databases, between January 2000 andAugust 2007. The following information was obtained from eacharticle: criteria for diagnosing diabetic neuropathy, patients’ aver-age age, antidepressant drug received and dose, sample size, dura-tion of the disease and treatment follow-up, outcome measure-ment, evaluation of pain and rescue medication. Results: A combined RR: 1.67 (95% CI 1.38–2.02) was obtained;this result indicated that the antidepressive agents studied wereeffective for controlling pain in diabetic neuropathy. The corre-sponding NNT for Duloxetine was established, according to ourinterests; NNT = 6 (95% CI 5–8) for achieving greater than 50%analgesia in patients suffering from painful diabetic neuropathy. Conclusion: Antidepressive agents are frequently employed in thespecific case of diabetic neuropathy; the combined result of thismeta-analysis has demonstrated their analgesic benefit.

- ÍtemSolo MetadatosBarriers to implementation of Evidence-Based Practice (EBP): a survey of Colombian and Spanish physiotherapists(2015-05-01) Girabent-Farrés, M.; Bagur-Calafat, C.; Serra-Llobet, P.; Sánchez-Puccini, M.B.; Domínguez-Sánchez, M.A.; Correa-Bautista, J.E.; Flórez-López, M.E.; Ramírez-Vélez, R.In countries such as Australia, Spain, United States, Colombia, and United Kingdom, the use, teaching and evaluation of ‘evidence-based physical therapy’ (EBPT) in clinical practice has been recently included in educational curriculum. As consequence, PT have support the EBPT, proposing a series of initiatives designed to generate, evaluate and disseminate research, and to implement these results in practice. Despite most research has focused on the use of evidence, including a critical review of studies and scientific literature in clinical practice, if it is desiderate that the use of EBP becomes a reality, it is important to examine the numerous perceived barriers to using research in clinical settings, despite the paucity of studies in this topic. This is especially true for professions such as physiotherapy that has a small research base and few resources compared with other professions. Typically, barriers are context-dependent; therefore, the implementation of strategies should be tailored according to the context and the specific barriers identified. The most commonly reported barrier among PT is the limited time, which constrains the identification and interpretation of research evidence, as well as the ability to apply research findings in clinical practice.

- ÍtemSolo MetadatosCapacity Utilization in Electricity Market Behavior: An Experimental Analysis(2019) Castañeda, Jaime Andrés; Arango Aramburo, SantiagoThis paper describes a laboratory experiment of electricity markets to study the effect of variable capacity utilization over market dynamics. Previous experiments have left out a potential source of cyclical behavior by assuming full capacity utilization. We run 12 experimental markets of the power generation sector, involving two treatments: a first treatment with fixed (full) capacity utilization where subjects make investment decisions, and a second treatment where subjects make both investment and capacity utilization decisions. Visual inspections, basic statistics and simulation tests run with decision rules estimated from experimental data show that the markets run under full capacity utilization present a cyclical behavior in market development. Conversely, such tests show a noncyclical market development for the markets run under variable capacity utilization, suggesting that varying capacity utilization favors stability rather than a cyclical behavior.
- ÍtemSolo MetadatosCase studies in the application of an effective anticollision risk management standard(2010) Poedjono, Benny; Avila, Carlos; Phan Van, Chinh; Isevcan, Erhan; Walker, John; Gonzalez, Luis Felipe; McCulloch, Simon; Juárez, Fernando; Moreno, Victor HugoEconomic, regulatory, and technological changes have steadily increased the oil and gas industry's need for an effective approach to assessing and managing the risk of well collisions. We have developed such an approach, which has been used successfully in recent years in a number of major land and offshore drilling projects worldwide. This approach has been adopted as a standard by the major oilfield service company of which we are a part. In this paper, we describe four recent experiences in which this standard has been applied, and discuss the challenges of its implementation and the economic, environmental, and health and safety benefits it provides. Based on its proven success, we make a case for using this approach as a foundation for development and adoption of an industry-wide anticollision and risk management standard.

In Heliconius butterflies, a small number of genes control the development of diverse wing colour patterns. Here, we used full-genome sequencing of individuals across the Heliconius erato radiation and closely related species to characterize genomic variation associated with wing pattern diversity. We show that variation around colour pattern genes is highly modular, with narrow genomic intervals associated with specific differences in colour and pattern. This modular architecture explains the diversity of colour patterns and provides a flexible mechanism for rapid morphological diversification.

- ÍtemSolo MetadatosEffect of a stretching program on the autonomic modulation in hemodialysis patients: pilot study(2009-05) Soler, Alejandro; Abaunza, Catalina; Zapata, Marcela; Povea, CamiloPURPOSE: To determine the effect of a stretching program over the autonomic modulation in chronic renal patients on hemodialysis. METHODS: An intradialysis exploratory descriptive pilot study was performed executing stretching exercises for one month every other day. The sample consisted of 3 men (53+/-13,2 yrs) and 4 women (26+/-5,7 yrs) who signed an informed consent. They were evaluated by means of a heart rate variability registry, a challenge of the autonomic nervous system, and stretching tests. The registry was divided into three phases with a 21 minute duration: an initial one in supine position, an orthostatic challenge, and a final one in supine position again. At the end of the month, the patients were re-evaluated. RESULTS: The standard deviation of the RR intervals showed the same tendency but in an opposite direction in women (29,9 +/-11,1 ms before vs 23,7 +/-12,6 ms after), with respect to men (15,3 +/-6,0 ms before vs 22,5 +/-5,0 ms after). The same result occurred with the total spectral power in women (2764,8 +/-1108,5 ms2/Hz before vs 1574,2 +/-1123,8 ms2/Hz after) with respect to men (1162 +/-437,4 ms2/Hz before vs 1646 +/-220,2 ms2/Hz after). The 30:15 index, which was abnormal in all at the beginning of the intervention (0,9+/-0,03), showed an increment in all patients and was normalized in one young female (1,05). The net differences of the high frequency index (-5,9+/-7,9 ms2/Hz before vs 3,9+/-8,9 ms2/Hz after) in the entire sample could indicate some degree of neurovegetative modification. CONCLUSION: This study concludes the existence of an autonomic dysfunction in chronic renal patients confirmed by the differences in the 30:15 index, and the tendency towards a modification in parasympathetic reactivity by means of a stretching program.
- ÍtemSolo MetadatosEffects of health professional supervised multimodal exercise interventions on cancer-related fatigue: systematic review and meta-analysis of randomized controlled trials(2015-05-01) Meneses-Echávez, J.; González-Jiménez; Ramírez-Vélez, R.; Correa-Bautista, J.E.Cancer-related fatigue (CRF) is the most common and distressing symptom in breast cancer survivors. Approximately 40% to 80% of cancer patients undergoing active treatment suffer from CRF. Exercise improves overall quality of life and CRF; however, the specific effects of the training modalities are not well understood. Purpose: This study aimed to determine the effects of health professional supervised multimodal exercise interventions on cancer-related fatigue through a systematic review and meta-analysis.
- ÍtemSolo MetadatosEstablishing normative reference values for the 20-meter shuttle-run test among schoolchildren in bogota, Colombia: the fuprecol study 2765 Board #288 June 3, 930 AM - 1100 AM(2016-05) Palacios-López, Adalberto; González-Ruíz, Katherine; Benavides Rodríguez, Lorena; Camelo-Prieto, Diana Lorena; Correa Bautista, Jorge Enrique; Ramírez-Vélez, RobinsonPURPOSE: There is increasing evidence that cardiorespiratory fitness (CRF) is an important health marker in youth. CRF values for children and adolescents from different countries have been published, but there is a scarcity of reference values for Latin American children and adolescents using recommended CRF estimation field tests such as the 20-m shuttle-run test as evidence of CRF in Colombian schoolchildren. In addition to presenting normative reference values, we also aim to establish the proportion of subjects whose aerobic capacity is indicative of future cardiovascular risk. METHODS: A total of 7244 children and adolescents (55.7% girls, with a sample age range of 9-17.9 years) completed the 20 m shuttle-run test (median age, in years = 12.8 (SD 2.3); 25th-75th percentile: 11.0-15.0. We expressed performance as the number of shuttle-runs completed and the estimated peak oxygen consumption (VO2peak). Smoothed percentile curves and tables for the 3rd, 10th, 25th, 50th, 75th, 90th and 97th percentiles were calculated using Cole’s LMS method. In addition, we calculated the number of participants who fell below proposed cut-offs for low CRF based on either completed shuttle-runs or by VO2peak. RESULTS: In general CRF levels increased with age. Among boys CRF, where higher between the ages of 14 and 17 and in girls between the ages of 12 and 14, but this increase was more modest. Shuttles and VO2peak were higher in boys than in girls in all age-specific groups. The proportion of subjects with a CRF indicative of future cardiovascular risk was 11.5%. By sex, 9.65% of boys and 13.1% of girls (X2 p < .001) displayed an unhealthy aerobic capacity in this study. CONCLUSIONS: Our results provide reference standards for sex- and age-specific twenty- meter shuttle-run test scores and VO2peak values in Colombian schoolchildren aged 9-17.9 years for the first time. These values are particularly important in public health and educational settings, and future research should establish a cut-off value for test performance that can predict present or future ill health.

Castellanos-De; Amaya-Amaya, J.; Molano González, Nicolás; Gutiérrez-Infante, F.; Anaya, Juan-Manuel; Rojas-Villarraga, A.There is a clear causal relationship between tobacco smoking and rheumatoid arthritis (RA), supported by observational studies and systematic reviews ; however, the effect of smoking on disease activity and the development of joint surface erosions has not been clearly established. Current data are conflicting, with some studies reporting an increased activity and progression of erosions and others showing an opposite relationship.

- ÍtemSolo MetadatosFRI0513 Does non - erosive rheumatoid arthritis exist ?(2013-06) Amaya-Amaya, J.; Calvo-Páramo, E.; Calderon-Páez, A.; Torralvo-Morato, G.; Calixto, O. J.; Mantilla, R. D.; Anaya, Juan-Manuel; Rojas-Villarraga, A.Background Erosions are the hallmark of rheumatoid arthritis (RA). Although most of erosions may occur within 2 years of onset of the disease, their progression does not correlate with the duration of RA. Moreover, information about non-erosive RA is ambiguous. Objectives To evaluate the prevalence and characteristics of non-erosive RA. Methods Since no agreement about the time-lag interval for erosions appearance in RA exists, and there is no universally accepted definition about it, a zero score in hands and feet X-ray at 5 years of RA duration (by SvHD) was arbitrary considered as cut-off. Therefore, patients meeting these criteria were searched in a large cohort of RA patients (ACR, 1987). Metacarpophalangeal and metatarsophalangeal joints as well as wrist ultrasound (US) imaging (by OMERACT), and computed tomography (CT) were performed to confirm non-erosive RA. Spearman correlation coefficient, kappa analysis and Kendall´s W test were used to analyze data. Results Out of a total of 800 RA patients, 38 met inclusion criteria (4.75%). Most of them were women (76.3%). The median (IQR) of the age and duration of the disease were 52 (13) and 10 (8) years, respectively. Rheumatoid factor (RF) and anti-CCP antibodies were positive in 86.8% and 55.3%, respectively. Non-erosive RA was confirmed by the three methods in only 7 patients. There was no concordance between the three methods. For this sample, the sensitivity and specificity were 70% and 52% for US, and 58% and 57% for CT, respectively. Non-erosive RA was positively although weakly correlated with improved functional class according to the HAQ (r=0.32, p=0.045) and ANA positivity (r=0.43, p=0.030), and negatively correlated with activity of the disease (by RAPID3, r= -0.32, p=0.05), the use of biological therapy (r= -0.31, p=0.05) and RF (r= -0.36, p=0.042). Conclusions Non-erosive RA is rare (0.87%). A good functional class, ANA positivity, RAPID3 score, the use of biological therapy and RF influence the absence of erosions in long-standing RA (i.e., 5 years). Since there is a high variability and a lack of concordance exists among the imagining techniques, a consensus and unified definition about non-erosive RA are warranted.

METHOD A sample of 12 graduate medicine students were subject to a follow-up of the number of hours of habitual sleep and underwent fMRI studies evaluating 4 paradigms before and after the PSR : Stroop (in standard and modified versions), right hand movement and verbal fluency. The PSR was equivalent to one night-shift, with at least 45 % sleep restriction with respect to the habitual sleep pattern during the previous week. The d2 attention test was done before and after the PSR to evaluate attention. RESULTS The d2 attention test did not show any significant differences between the habitual sleep condition and the PSR. Significant differences were found in BOLD activity for the motor function (P<0.05). Changes in BOLD activity were not observed for the Stroop nor verbal fluency paradigms.

The disease is irreversible and incurable; therefore, early diagnosis or prediction may become useful to control CWP. Oxidative stress is known to play an important rolein pathogenesis.Oxidative DNA damageis repaired by the Base Excision Repair (BER) pathway; therefore, genetic variations in BER pathway enzymes may confer differential risk in CWP. On the other hand, as has been shown for other environmental pollutants DNA methylation may represent an interface between exposure and disease. Biomarkers of exposure, effect and susceptibility were assessed in order to evaluate oxidative stress and methylation changes associated with the disease and the influence ofOGG1and XRCC1polymorphisms in CWP development. A group of 474 underground coal miners were selected to participate in this cross-sectional study. This population was subdivided into two groups of healthy and CWP miners. We show that underground miners who develop pneumoconiosis have a higher level of the oxidative stress marker 8-OHdG in white blood cells, and that this oxidative DNA damage was associated withXRCC1194Trp allele. In addition, leukocytes from CWP cases are differentially methylated at two immune-related loci when compared with control miners, as assessed by bisulfite pyrosequencing. Information regarding genetic, epigenetic and exposure conditions will contribute to the better understanding of disease aetiology in a population.
- ÍtemSolo MetadatosGenetic-molecular study of the egfr/ pi3k/akt pathway in high and low grade gliomas(2010-11) Erira, Alveiro; Penagos, Jose; Zubieta, Camilo; Velandia Hurtado, Fernando Alfredo; Arboleda, Humberto; Arboleda GonzaloGliomas account for 70% of all central nervous system (CNS) tumors and are classified according to the WHO in low (I and II) or high (III and IV or glioblastoma multiforme, GBM) grade malignancies. The survival of patients with a high grade is less than 2 years. The EGFR/PI3K/AKT pathway promotes cellular survival, inhibits apoptosis and differentiation. This pathway is regulated by phosphatase homologous of tensin (PTEN). In gliomas, amplification of EGFR and PI3K has been reported, as well as mutations in PI3KCA and PTEN, which are associated with permanent activation of this signaling pathway and with decreased expression of pro-apoptotic genes such as Bax and increased expression of anti-apoptotic genes such as Bcl2. We obtained 30 samples of gliomas with high and low grade. Amplification was assessed by real-time PCR with TaqMan probes for the EGFR, PI3K and AKT genes. Mutations in PI3KCA (9 and 20 exons) and PTEN (5 and 6 exons) were determined by direct sequencing from DNA using specific primers for each exon. The results of each sample were analyzed in 50 e30 and 30 e50 orientations using the program FINCH TV. Subsequent alignment was performed for each sample with the human reference sequence (obtained from Gene Bank) using the GENE RUNNER program. To assess the expression levels of the PI3K, AKT, Bax, and Bcl2 genes, cDNA obtained by reverse transcription was amplified by real-time PCR with SYBR Green using specific primers for each gene. The results were analyzed by calculating the CT. We did not find any high level EGFR amplifications. Only one grade IV sample showed an increase in gene dosage (2.8 times). PI3K was found amplified in 50% of grade I and II (0.4 times) and in 45% of grade III and IV (between 0.5 and 0.9 times). However, in 18% of the grade IV samples, the increase was greater than 1. AKT was not found amplified in the samples analyzed. Also, no mutations were found in PI3K and PTEN. In low-grade gliomas we found an increase (O1 time) in PI3K (62%), AKT (12.5%), Bcl2 (50%), and Bax (25%) gene expression. In highgrade tumors over-expression of PI3K (50%), AKT, Bcl2, and Bax (all 32%) was found.
